PHYSICIAN DRUG MONITORING PROGRAM
North Dakota Physicians and physician assistants, among others, may now obtain a profile for patients currently under care that contains all controlled substances, including the non-scheduled drugs tramadol and carisoprodol, that have been dispensed to a patient in North Dakota since January 1, 2007.

The Patient Profile Request Form may be accessed in the attached PDF file and faxed or mailed to he North Dakota Board of Pharmacy.
